The Oklahoma Insurance Department (OID) announced it has fined and revoked the license of resident insurance producer Leslie Clark of Stigler, Oklahoma, following an investigation led by its Anti-Fraud Unit.

On February 11, 2026, the OID office held a hearing before an independent reviewer, but Clark was unable to attend. Clark was found to have improperly withheld and misappropriated 18 cash receipts from consumer premiums totaling $7,269.38. OID suspended Clark’s driver’s license and fined her $2,000.

The case began when Farmers Insurance Group filed a complaint on December 5, 2024. The complaint alleges Clark failed to forward premiums to the company.

Farmers’ internal investigation revealed that between August 23, 2023, and June 12, 2024, Clark had entered 18 cash receipts into its internal payment system to credit insureds’ policies. Clark failed to transfer funds to farmers.

Farmers’ investigation also found that Clark’s personal homeowners policy was canceled on June 23, 2024, due to non-payment. Clark entered a check receipt for $6,885.00 on July 31, 2024, into the insurance company’s internal payment system to reinstate his homeowners policy, but failed to deposit the check into Farmers’ bank account for the amount of the check.

Farmers recovered their receivables and consumption policies were not affected.
